2. Covid Return To Football Parental Consent

Following the FA guidelines issued in June 2020, parents/guardians are required
to provide written consent for any child wishing to participate in football activity
during the Covid-19 pandemic.

It is essential everyone is clear about what is expected of them, and that they
agree to adhere to the Covid-19 guidelines and work together to manage social
distancing and strong hand hygiene.

Parental consent is given electronically via your Pitchero account and is required
for ALL new and existing players. Consent only needs to be given ONCE and
must be done 24 HOURS BEFORE your child intends to participate in any club
activity (training session, match etc). This is to ensure the information we hold is
up to date and in compliance with the FA’s Covid-19 policies.

If a child does not have parental consent, they will not be allowed to participate
in any club activity. Team Managers will be given a weekly monitoring list, so
they are always aware of which children have parental consent, and which do
not.

By giving consent, you agree :

• That the named player may participate in activities arranged by Northowram
• Junior FC;
• That you have read the Covid-19 guidelines outlined in this document
• (including the FA guidelines);
• To discuss the guidelines with your child, so they are aware of the relevant
• safety procedures being implemented by Northowram Junior FC;
• That your child will self-screen, using the symptom checklist on page 11,
• before travelling to any club activity. You agree to keep your child at home if
• they are symptomatic or have been in contact with another person with a
• suspected or confirmed case of Covid-19;
• To keep your child at home if they have been asked to self isolate by their
• school or the NHS Test & Trace system;
• If you have any concerns regarding the guidelines and procedures that you
• will bring them to the attention of a Club Official immediately, and you should
• not allow your child to participate in any club activities until you are happy.
